LINE 2: An In Case of Emergency contact number – ICE 1: 082 123 4567
LINE 3: A second In Case of Emergency contact number – ICE 2: 082 890 1234
LINE 4: Medical aid name and number (i.e. Discovery 1234567)
LINE 5: Any Allergies and/or Medication
LINE 6: You do not need to complete this line, but it is available if you have more info to complete i.e. Blood type 0+
