These helmet tags are also ideal and can fit equestrian helmet straps and the like.

- LINE 1: Name and Surname
- LINE 2: An Emergency Contact Name & Number
- LINE 3: Medical Aid Company and Number (i.e. Discovery 1234567)
- LINE 4: Medication/Allergies
- LINE 5: Blood Group/Organ Donor
- LINE 6: You do not need to complete this line, but it is available if you have more info to include
